Further informations about this job opportunity kindly read the description below. The Transitional Care Manager (TCM) will visit members in designated hospitals. This is a field based position with a hospital component. ! The TCM will manage the member’s transition from hospital to home. They will complete health assessments, post hospital assessments, assist with scheduling follow-up appointments with providers and coordinate patient care. The TCM will provide clinical and medical management services, including case management, care coordination, health assessments, interventions, and may assist with discharge planning. Work is generally fast paced, self-directed and not prescribed; this person will work from home when not completing face-to-face patient assessments in a hospital setting.
The territory for this position is Madison County
Primary responsibilities:
Engaging members in person in a hospital setting.
Travel to assigned hospitals and other hospitals as needed
Coordinate follow-up appointments with provider, coordinate other services, assess treatment needs, referrals to community resources
Functioning as an advocate for members and ! decreasing unnecessary hospitalizations and emergency room vis! its
Delivering a holistic approach to coordinated care based on the member’s needs
Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file
Creating, reviewing and revising care plans
Collaborating with the member's PCP to deliver and coordinate necessary services
Building relationships with members and their families; assisting them with proper health choices
Ensuring cases are documented in a timely manner
***If you do not have a current CCM, you will need to obtain one within 2 years of start date***
Requirements:
Current residence in the State of Tennessee
Current, unrestricted RN license in the State of Tennessee
1+ years of Home Health or other Case Management experience
1+ year of clinical experience
Field experience
Level of proficiency with Microsoft Office, with the ability to navigate a Windows environment and ability to type and talk at ! the same time
Strong organizational skills and multitasking abilities
Reliable transportation with appropriate insurance, and the willingness to travel in territory
Additional Preferred Assets:
Bachelor's Degree in Nursing
Certified Case Manager (CCM)
Prior experience working in a health plan or other managed care setting
Previous experience with the Medicaid population
