Role Summary
Responsible for assisting in the process of responding to referrals and other requests for services and information. Provides ! telephonic risk screening to members with mental health disorders. Performs outbound calling to members to ensure follow up post discharge after hospitalization. Provides technical assistance to the discharge planning process and general support to the case management program.
Responsibilities
Responsible for answering phones and providing customer service to mental health patients. Handles standard outpatient referrals and screening for mental health risk. Works with patient to schedule post hospital follow-up visit and verify adherence to treatment plan. Responsible for front end case management screening. Performs case escalation to the clinical team on the basis of a series of risk identifiers. Supports providers in establishing on-going treatment plans for their members. .
Requirements
Qualifications
Minimum: Bachelor’s degree in a social sciences field. Previous call center/customer service experience, strong commu! nication skills and computer data entry skills.
Pr! eferred: Three or more years related experience. Master’s preparation in a social science field. Bilingual Spanish / English strongly preferred.
