Intake Coordinator

The Intake Coordinator will care for patients in their homes, build relationships and will share the challenges and triumphs of day-to-day life.
A licensed nurse to assist in coordination, and pro! vision of quality home nursing care while demonstrating fiscal responsibility and maintaining the highest standards of care and ensuring compliance with all policies, procedures and regulatory requirements.
Coordinates communication of information relevant to the patient care process.
May schedule patient care staff, receive referrals and physician orders and perform patient visits and all appropriate documentation to the management and provision of patient care.
Qualifications
Current and unencumbered state license to practice as a Registered Nurse.
One year of clinical experience as a nurse.
Strong computer and software skills.
Demonstrated knowledge of the appropriate skills for communicating with individuals of all ages. Excellent interpersonal skills including verbal and written communication skills.
Current CPR certification, preferred.
Demonstrates knowledge of Federal/State rules and re! gulations, ACHC standards and other regulatory requirements.
Valid driverâs license, reliable transportation and required liability insurance.
